These checklists are to be used as guidance for new employees or employees separating from the current department or from Indiana State University. It is intended to provide you with key security and data storage items to check in order to successfully complete these important processes.

Only Faculty or Staff may submit a Needs Consultant Appointment Request.

Onboarding or Separation Checklist - Need Consultant Appointment Request
This request requires basic account holder information. Specifically, this includes the following account holder items:
- Employee Full Name
- Employee Job Title
- Start Date or Separation Date
- Special Concerns for Employee Position
- Availability for Consultant Visit
Once the request as been submitted, the requestor will be contacted to confirm the consultant appointment date and time.
