Qualtrics Survey Consultation


Qualtrics is a cloud based survey tool with a user-friendly interface. Users should be able to create a basic survey in Qualtrics with a relatively shallow learning curve.

Standard Service Features

  • MDRPS provides assistance with advanced features in Qualtrics, under the assumption that the user already knows the basics of how to create a Qualtrics survey.
  • Some advanced features include, but are not limited to, general layout design, display logic, survey blocks and survey flow, distribution panel, customized HTML tags and Javascript, and embedded graphics.

SLA: You can expect this service to be completed within 30 business days.


  • Only Faculty can request this service.
  • Customers are expected to have the electronic versions of survey questions and answers ready, and compile your own distribution list when applicable.
  • Customer has the responsibility to distribute the survey, collect, and analyze survey responses. 
  • Customer is responsible to obtain IRB approval when applicable.


Provide the following information when requesting the consultation:

  1. Purpose of the survey.
  2. Desired survey release date.
  3. Attach (if ready) the survey questions in a Word document.
