Desktop Software Applications - Report an Issue

Description

If any installed or web-accessed Microsoft Office or other productivity type software is not functioning as expected, you will use this form to report the issue. 

Standard Features

Based on the information provided in the request form, OIT will route the issue to the appropriate personnel and troubleshooting team.   Specifically, you can report issues with the following applications:

Note: You can submit problems with other applications not listed as well.

SLA:  For non-urgent issues you can expect a reply within 4 business hours. Stop Work/Urgent request you can expect a reply within 15 minutes.

Directions

  • Before submitting a software issue ticket, we recommend restarting your computer.  A computer restart will resolve many temporary issues.  However, if the issue recurs after a restart, whether immediately or after a short period of use, OIT will need to evaluate the issue.
  • If the software application is not listed select "Other" when prompted and provide the name of the application.
