Introduction
Surveys can be built in Canvas- New Quizzes to allow instructors to collect student feedback or responses. Surveys can be configured to collect anonymous or non-anonymous feedback, graded or ungraded, to help meet different instructional goals. Instructors also have access to new Survey data within the ‘Reports’ tab, providing more detailed insights into survey results.
Create a New Survey
In your Canvas course, select Quizzes from Course Navigation.
Click +Quiz/Survey
At Choose a Quiz Engine, Select New Quizzes/Surveys, and Click Submit

In the Quiz Type dropdown menu, select either Graded Survey or Ungraded Survey

NOTE: Ungraded Surveys have an Anonymous Submission option

Complete the survey settings information (Assignment Name, due date, etc.)
Click Build at the bottom of the screen to add questions or Save to return to the survey at a later time

Add Survey Questions
Add Survey Questions using the same steps to add Quiz Questions. Click the blue + sign to add questions.

Choose the question type that best fits your survey
Survey Questions are saved automatically
Remember to publish the Survey in order for students to see and complete it.
Review Survey Data in Reports Tab
To review Survey results, go to the Survey settings and click Build

Select the Reports tab

Individual responses are available by selecting the Moderate tab

Important Notes

Surveys will not evaluate correct or incorrect answers for essay questions

Participation points are optional

Item Banks can be used to reuse survey questions across courses but do not use the Item Bank questions for repeated questions in the same quiz
